- Determination of the Treatment Plan, through a specialized evaluation of each case by a highly qualified interdisciplinary team.
- Coordination and scheduling of appointments (diagnostic tests, procedures, consultations, surgeries).
- Periodic update in English or Spanish of the patient's health status to the referring physician and/or Responsible Payment Entity (Insurance Company).
- Permanent communication with the patient and his/her family regarding the evolution of the treatment.
- Coordination of the post-operative care plan. Patient education plan about healthy lifestyles, disease management, and discharge care plan.
